Early Years and Childcare - Early Support

The Early Support Coordinator -coordinates the Early Support pathway/the pathway is for preschool children with complex health/learning/care needs and which has now extended to include children with complex health/learning/care needs in their reception year.

The pathway is accessed as a professional referral in via the completion off an Early help assessment/and referral to the Early Support referral tray -once completed the gateway procedures

Early Support cases are discussed at Multiagency meetings and recommendations are made to support the identified needs.

Early Help

The Early help team is a separate team within the Council that run the Early help processes /they can offer you support with a wide range of things from how to complete an Early help assessment/training on the Early help electronic system/supporting you with TAF meetings- Please see the SEND Information Hub for information on the support and services that the Early help team operate.
